• Call the Adult Care Center at 434-847-8111
• Request an Inquiry Packet and talk to our Registered Nurse or Executive Director
• Schedule a FREE Assessment Visit
• Return completed application, physician's statement, & other forms
• Consult with Center staff to develop an individualized Plan of Care
• Complete enrollment agreement and payment arrangements

Participants enroll on a scheduled basis - attending from one to five days per week. 

Enrollment is based on participant meeting admission criteria including:

• Able to benefit from a social and group program
• Able to communicate their needs
• Substantially continent

Although the Adult Care Center recognizes the need for care for many individuals, we are unable to offer services to those who may:

• Act aggressively or disruptively to other participants
• Routinely seek to leave the facility
• Require consistent one-on-one staff attention
